Alamance Regional Medical Center serves as a site for nursing, clinical and high school students to learn more about healthcare careers and complete clinical rotations. On this page you will find all the forms and orientation materials needed to begin. You will be required to complete this information before beginning your rotation, class or visit.

For schools of nursing, clinical site priority is given to members of the Joint Committee for Clinical Resource Planning (JCCRP).

Additional clinical and professional site placement for students currently enrolled in a program of study is available on an individual basis. The program of study must initiate or verify a current contract with Alamance Regional Medical Center prior to the learning experience.

Schools of nursing should maintain documentation of the following requirements:

  • Social security verified criminal background check
  • 12-panel urine drug screen
  • American Heart Association CPR for Health Care Providers
  • CDC recommended immunizations/titers
  • Current TB screening

Please download, print and complete these Student Forms (four pages).

If you will be administering medications, you will also need to download, print and complete the System Access for Pyxis Form.

Please note that students who are part of a group should submit all forms to their instructor. Completed forms should be submitted to the Alamance Regional Training and Development Department in person or you may scan and e-mail the forms to hubblori@armc.com.
