Bringing Healthcare Home
Continuous Care services are provided for patients who need long-term care. These services are designed for chronically ill adults, as well as those who are healthy but need assistance to maintain the level of independence they desire. The goal of Continuous Care services is to support the patient, promote independence in self-care, and improve quality of life. We also provide supplemental staffing for health-care organizations and other facilities.

All Continuous Care services listed below are available in Alamance, Caswell, western Orange and Guilford counties. We also provide HIV services in Rockingham County. These services are available for patients who have been released from any area hospital; however it is not necessary to be hospitalized before receiving continuous care services.

In-Home Aides
In-home aides work under the direction and supervision of a registered nurse. Aide services include:

  • Helping the patient maintain personal hygiene
  • Assisting the patient with walking and moving around
  • Helping maintain a healthy, safe environment
  • Reporting any change in the patient’s mental or physical condition, or home situation
Nursing Care
A variety of nursing services are available. These services include:
  • Performing lab tests ordered by the physician
  • Performing physical and environmental assessments
Community Alternatives Program for Children
The Community Alternative Program for Children (CAP/C) provides case management services for medically high-risk children who are at risk of institutionalization. This is a Medicaid based program.

HIV Services
HIV Services are provided for individuals infected with HIV. These services include HIV case management, benefit advocacy and treatment adherence.

Physical and Occupational Therapy
Licensed physical and occupational therapists provide rehabilitation services and home safety assessments.

Pastoral Care
Clinically trained ministers serve as home health chaplains for patients or families who request these services. These professionals help patients and families use their beliefs and values to aid in recovery and find meaning in the midst of illness.
