Medication Assistance Program (MAP)

• Available to the citizens of Alamance County and surrounding communities who are currently uninsured and live at or below 250% of the federal poverty level
• Must be 18 years of age or older
• Improve patient access to medications by using drug manufacturer’s patient assistance programs
• Help people avoid and manage any gaps in prescription coverage, such as reaching the Medicare D “doughnut hole” and maximum drug benefits for private insurance

Medication Therapy Management (MTM)

• Available to citizens of Alamance County and surrounding communities
• Must be 18 years of age or older
• Provide education to patients about their medications and medical problems
• Maximize the benefits of each medication including cost and effectiveness
• Communicate with doctors and other health care providers to maintain quality care

Disease State Management (DSM)

• Available to citizens of Alamance County and surrounding communities
• Must be 18 years of age or older
• Help patients understand how to manage their health conditions
• Monitoring and managing chronic health conditions such as high blood pressure, diabetes, cholesterol and smoking cessation

Cost Assessment

• Available to all citizens of Alamance County and surrounding communities
• Must be 18 years of age or older
• Review cost of current medications
• Provide you and your doctor with recommendations to lower your cost of medications
