Medication Assistance Program (MAP)
- Available to the citizens of Alamance County and surrounding communities who are currently uninsured and live at or below 250% of the federal poverty level
- Must be 18 years of age or older
- Improve patient access to medications by using drug manufacturer’s patient assistance programs
- Help people avoid and manage any gaps in prescription coverage, such as reaching the Medicare D “doughnut hole” and maximum drug benefits for private insurance
Medication Therapy Management (MTM)
- Available to citizens of Alamance County and surrounding communities
- Must be 18 years of age or older
- Provide education to patients about their medications and medical problems
- Maximize the benefits of each medication including cost and effectiveness
- Communicate with doctors and other health care providers to maintain quality care
Disease State Management (DSM)
- Available to citizens of Alamance County and surrounding communities
- Must be 18 years of age or older
- Help patients understand how to manage their health conditions
- Monitoring and managing chronic health conditions such as high blood pressure, diabetes, cholesterol and smoking cessation
Cost Assessment
- Available to all citizens of Alamance County and surrounding communities
- Must be 18 years of age or older
- Review cost of current medications
- Provide you and your doctor with recommendations to lower your cost of medications
